The application for the 2009-2010 school year is now available online for downloading and printing in PDF format.

Please follow all instructions when completing the application. Applications that arrive incomplete or filled out incorrectly will not be considered.

Applications sent by fax or delivered by hand will not be considered.

Please note that the application deadline is March 1, 2009. 

The first five pages are instructional and will help you properly complete the necessary paperwork.

• Page 1 of the application includes a history of the SSP program and a list of current SSP Partner Schools.

• Pages 2-3 will help you determine if you meet SSP's eligibility requirements and provide general information about the application process.

• Pages 4-5 give directions that will help you complete the 4 written parts of the application.
